Home Care Assistance

I Need NDIS Support

  • Hourly Care
  • 24 Hour Care
  • Hospital to Home
  • Specialised Care

How do I get NDIS support?

Eligibility criteria exist for NDIS.  You can check your eligibility on the NDIS website at https://www.ndis.gov.au/applying-access-ndis/am-i-eligible

Once you have determined you are eligible, you can then apply for funding.  This involves completing an Access Request Form and submitting this to the NDIA.  Your GP can help you complete this form, or you can get help from an NDIS Local Area Coordinator. Visit https://www.ndis.gov.au/how-apply-ndis/what-access-request-form for more details on the application process.

How We Help

What is a NDIS Plan?

Once the Access Request Form is received, a planning
meeting will be organised to discuss your goals and the
activities/asks you wish to receive support for. This will form your NDIS Plan, which needs to be approved by the NDIA prior to being actioned.

The plan is approved for a 12-month period and will be reviewed six weeks prior to the end of that 12-month period. This is done in order to reflect on the last 12 months achievements, activities and to understand future goals and needs. The plan review meeting can be done in person or over the phone.

Things to consider when preparing for your NDIS plan

Each person living with a disability has different needs and NDIS funding is there to help with short and medium, to long-term goals in their NDIS plan. Before your plan meeting, it is advisable to consider:

  • What areas do you need support?
  • Who do you want to manage your funding? The NDIA or a Plan Manager can do this for you or you can choose to Self-manage.
  • Do you need a Support Coordinator to help coordinate support and services or will you do this on your own?

What does NDIS funding cover?

NDIS funding is based on what is ‘reasonable’ and ‘necessary’ to achieve the goals outlined in the NDIS plan. There are three types of support budgets that may be included in your NDIS plan.
  • Core Supports Budget – includes assistance with activities of daily living, social and community access
  • Capacity Building Budget – supports the capacity to participate more fully in the community
  • Capital Support Budget – relates to supports like assistive technology or modifications to your home

Home Care Assistance run by Jeremy and Samantha, a lovely couple with a caring attitude and a family themselves. After bringing my Mum to Sydney and needing expert help immediately I found HCA online... and liked their story. As soon as they walked in our door, we knew we were safe. I liked the fact that there was direct contact with the owners and nothing was too much trouble. The carers selected for Mum were perfect for her and very capable. Unfortunately Mum has had to move to a nursing home now but I would happily recommend this wonderful business to anyone with aged care needs.read more
